I have worked with brain injury, autism, dementia, alzheimers, rehabilitation, cancer in various areas of the body, kidney failure, hospice. My heart has been in this field since I was a very young age, and I have continued pursuing it throughout the years. I like to think those providing care for start as a patient/client and end up as family. I have empathy for each individual and always open to new ideas that may help the clients wants and or needs. I believe in always keeping family in touch with progress or possible declines, proper documentation and record keeping. I
Shannon Jacobs has been a blessing to our family ever since our grandfather Eddie became ill. Shannon always made sure his pill box was full and reminded him often about upcoming doctors appointments. She was loved by Eddie and was very helpful in his care, It did not matter what he requested she always assisted him any way she could.
Shannon helped by being very thorough and organized with her work. She was very dependable and professional. You could also tell that she cared when working and that made a big difference to me.
